Private Dentist Report

Can be completed by child's private dentist for dental exam due in grades K, 3, and 7 and/or for any new student in school district as needed. 

Private Physician Report

Can be completed by child's private physician for physicals due in grades K, 6, and 11 and/or for any new student in school district as needed. 

Consent for Medication Form

May be completed by child's prescribing medical provider for any medication administration necessary during school day. 

Field Trip Medication Administration Delegation Form

Can be completed if you prefer that a designated adult (chaperone or staff member) to administer your child medication in the absence of a school nurse on a school field trip.
