Were you excluded from play due to serious injury or illness and required medical treatment from a licensed physician of medicine or osteopathic medicine? If yes, then a Section 8 form must be signed by an MD/DO and returned to the proper athletic personel. Section 8 Form: Physican Recert following Injury (Section 8)

PRE-PARTICIPATION FORMS

Before any athlete can participate in a sport, athletic packets must be completed and turned into the Athletic Office.  A physical examination must also be completed by your family physician after May 1st of the up-coming school year.

NEW ATHLETES has not played a school sport this school year

  1. Halifax Required Pre-Participation Athletic Forms

    • Includes acknowdledgement and understanding of:

      • Acknowledgement that I have read the Student Athlete Handbook for the current school year and understand it’s policies.

      • Halifax Area School District Concussion and Traumatic Brain Injury Information Sheet

      • Consent to Release Athlete’s Photo and Other Information

      • Halifax Area School District Activity Drug Testing Consent Form

      • Assumption of Risk and Acknowledgement of Warning Form

  2. Pre-Participation Physical Form (Sections 1-6) (Archery does NOT need Section 6)

  3. ImPact Testing (Conducted seasonally by the Athletic Director and Athletic Trainer for all 6th, 7th, 9th, and 11th grade athletes)

  4. Participation Fee of $35 (annual fee)

RETURNING ATHLETES has played a school sport this school year in a previous season
